Please note this reorder form option should be used only for patients who are currently using a V.A.C. provided by SunMED Medical. If you would like to place an order for a new V.A.C., please contact us @ 855-304-2930. NOTE: Contact SunMED directly @ 855-304-2930 for any same day requests.

Per Medicare guidelines, if this patient remains on the VAC longer than four months, a face-to-face appointment with a physician is required for continued use of the VAC for the fifth and any subsequent months. If the patient in non-compliant with following Medicare's guideline, the VAC treatment will be discontinued.
