SunMED Medical Solutions is a national provider helping patients obtain replacement external devices through insurance when they are lost, stolen, or damaged due to negligence.
How does it work?
Step 1: Submit an online request for replacement through SunMED with the following information:
- Your implant model number and implant date, found on your Medtronic
Medical Device Identification Card
- Note: Your patient ID card may not look exactly like this card
If you cannot find your card or locate this information, please call Medtronic Patient Services at 800-510-6735 to confirm you are ordering the correct device. Single patient use devices cannot be returned.
- Prescribing doctor name and phone number
- Your insurance information (or payment method if bypassing insurance)
Step 2: SunMED will work with your prescribing doctor to obtain a prescription and documentation describing your need for the device from your medical record as required by your insurance. If your prescribing doctor is unaware of your programmer being lost, stolen or damaged, or non-functioning please contact them right away to let them know that SunMED will be requesting the documentation your insurance requires to cover your replacement.
Step 3: SunMED will submit a prior authorization request to your insurance (this typically takes 7-10 business days). A member of our SunMED customer care team will contact you to review insurance coverage, including collecting any out-of-pocket cost prior to shipping. We will never surprise you with a bill.
